Ordering
Purchase orders need to be completed and faxed to 1-978-229-9585 or emailed to csusa@sirtex.com no later than Tuesday afternoon of the week before the treatment is scheduled. In order for Sirtex to be able to ship doses to a hospital, the center needs to have a Sirtex-approved radioactive materials license which lists SIR-Spheres microspheres or a comparable statement as authorized by the respective state. In addition, hospital contact information must be kept current. Please visit the Starting a SIR-Spheres microspheres program section for more information. Treatment days of the week are: Tuesday, Wednesday, and Thursday. Doses are delivered on the day of scheduled administration. Dose availability may be affected by holidays.
Cancellation
Doses which cannot be administered for clinical reasons may be cancelled. Cancellations should be made promptly and must be confirmed in writing.
